JaxCareConnect is a program of WeCareJax.
We were created by the local nonprofit safety net of free and charitable clinics to assist uninsured residents of Duval County in finding a primary care medical home. In addition to establishing primary care, we evaluate the whole person and explore other barriers to care in order to support best possible health outcomes.
Navigating health care can be overwhelming. The JaxCareConnect team is here to compassionately serve our neighbors and provide guidance about access to a provider. Our Patient Health Advocates work individually with each client to connect them to a primary care doctor, navigate community resources, and provide ongoing support along their healthcare journey.
JaxCareConnect aims to better inform uninsured neighbors about their health care options so that all Duval County residents have access to comprehensive primary care — regardless of insurance status or ability to pay.
