Keep Living at Home is a Class A licensed Home Healthcare Agency. We specialize in customized care plans specific to your needs with a family portal that can be viewed in real time for families far away. We work alone or alongside agencies providing skilled services or hospice companies to make sure that every client / patient is receiving wrap around services. Our services can be booked by the hour or up to 24 hours/day. We have in home support services such as showering, dressing, ALD assistance, medication reminders, exercise assistance, cooking, cleaning, shopping, or errands. We also offer a concierge service for those people that don’t need hands on services but perhaps a one-time need or a weekly service such as cleaning, shopping, planning, transportation etc.
New in September 2021 we will be providing two additional services. A Nurse Navigator and a Hospital / Rehab to Home Program.
Our Nurse Navigator is to assist families by having one of our RN’s assist your family member to their appointments and address questions that you would like to know, as well as assisting with those more difficult decisions like when the right time is to move. This is also a service that we have been providing for our clients for many years, and now we are going to open it up to anyone that may need a little more guidance or assistance on how to help their parents or grandparents.
Our Hospital to Home and Rehab to Home program is also a service that we have been doing for our clients for many years but have seen the need for this to be offered as a one-time service to anyone. After being in-patient for even a week there is often a lot more to think about in terms of returning home. We will meet with you in the hospital / rehab facility and provide a list of post-discharge needs as well as things to think about and assist you with any of those needs if necessary.
