LIFEPlan is a Care Coordination Organization (CCO), formed in 2018 along with six other CCO’s in New York State to meet the needs of people with intellectual and developmental disabilities by collaborating with service providers through a Health Home model. CCOs are staffed by Care Managers with training and experience in the field of developmental disabilities.
LIFEPlan CCO is a partnership of community-based agencies with a long history of serving people with I/DD, providing advocacy, recreational, educational and vocational services. We share a person-centered focus that improves services and supports through networking with highly trained human service, healthcare and other specialists.
