As a dedicated Support Coordination provider, we assist individuals with disabilities in navigating the National Disability Insurance Scheme (NDIS) to maximize their plans and access the right services. Our goal is to empower participants by providing tailored guidance, ensuring they build capacity, exercise choice and control, and achieve their personal goals. We offer three key services: - Support Coordination – Helping participants understand and implement their NDIS plans, connect with service providers, and develop the skills to manage their supports independently. - Specialist Support Coordination – Providing expert assistance for individuals with complex needs, ensuring they receive specialized support to address barriers and achieve positive outcomes. - Recovery Coaching – Supporting participants with psychosocial disabilities by promoting resilience, self-management, and recovery-oriented approaches. Our team is committed to delivering person-centered, flexible support through both face-to-face and virtual options, ensuring accessibility and convenience for all Self-Managed and Plan-Managed participants.
