Looking Ahead Inc.’s trained and certified advance care planning (ACP) specialists—all medical professionals—deliver ACP services and education to all members of the community regardless of health or income status. The 501c3 community service organization is supported by grants, contributions and individual fee-for- service clients. Advance care planning is the process during which individuals consider, discuss, document, share and file their personal healthcare choices for loved ones and healthcare providers to follow should they be unable to speak for themselves. These choices are recorded in a document called an advance directive, which incorporates both a living will and a durable healthcare power of attorney naming the individual’s healthcare agent. Completing and sharing these documents is the best way to ensure that a patient’s wishes for care will be known, understood and carried out by those making decisions on the patient’s behalf.
