Our unique service model of People + Technology enables practices to quickly and easily implement a Medicare-compliant CCM program that will allow you to provide high quality care to your qualifying patients and increase revenue for your practice — without investing in new staff or office space. As you may know, Medicare's CCM program allows practices to bill CMS for the extra attention and care coordination beyond office visits that you provide to your Medicare patients living with 2 or more chronic conditions.
