To serve elderly and aging individuals with disabilities by understanding cultural differences and providing support in the areas of language, literacy, and culture to enhance community and personal relationships in a manner that promotes independence, understanding, dignity and quality of life.
Our Philosophy

To provide the community with a multidisciplinary team of diverse nurses and social workers to assist there maturing populations by decreasing hospital admissions, increasing medical appointment compliance, enhancing adherence of medical treatment regimens, increasing community involvement and awareness of referrals and resources, enabling support and understanding, early detection of infection or illness, all through cultural appreciation and language.
