About Michigan Collaborative for Type 2 Diabetes (MCT2D)

The Michigan Collaborative for Type 2 Diabetes (MCT2D) is a population health Collaborative Quality Initiative that seeks to engage and empower medical providers across Michigan to accelerate dissemination and implementation of evidence-based strategies to prevent or slow disease progression of Type 2 Diabetes. Established in 2021 with funding from Blue Cross Blue Shield of Michigan Value Partnerships, MCT2D has members from across 230 primary care, endocrinology, and nephrology practices, over 1000 participating physicians, represented by 28 physician organizations. The goal of MCT2D is to support Michigan providers to deliver high quality behavioral and medical treatment to prevent, slow or reverse the course of type 2 diabetes. Given the persistent disparities in prevalence and quality of care, our efforts to disseminate effective strategies aim to specifically address social determinants of health and to support providers who are working in low resource, high need environments. OUR THREE INITIATIVES 1.) Support lower carbohydrate eating. 2.) Expand access to and use of continuous glucose monitoring (CGMs) for type 2 diabetes. 3.) Remove barriers to prescribing newer classes of drug treatments for type 2 diabetes, specifically: GLP-1 & GIP receptor agonists and SGLT-2 inhibitors.
