This process may be initiated through a number of 100% Medicare-paid services including the Welcome to Medicare Exam, Annual Wellness Visit, LEAN (Lifestyle Education And Nutrition) Weight Loss Program, Behavioral Therapy for Cardiovascular Disease, and various screening and diagnostic tests.We also offer the SWEET LIFE Diabetes Education Program covered under Medicare. For those with-out Medicare, we accept insurance when coverage is available. We work in conjunction with your other doctors and share any data you feel is appropriate.
