Our member practices have come together to form a PCN so that as a network we can:
• Support patients on a more local level;
• Create services that meet the needs of our patients on a more bespoke basis and support capacity within our practices;
• Support practice resilience, reduce costs and reduce pressures in practice;
• Develop and design localised care pathways and multi-disciplinary teams to improve patient experience, patient care, the wider system and to generate income which can be re-invested into patient care locally;
• Work together to design and deliver services that are specific to primary care needs at the current time;
• Improve and enhance the current workforce models.
Working together enables us to develop and deliver more efficient services for patients across the District and look at new ways of working.
