Community Health Partners is a community-based organization that focuses on the continuum of care across a spectrum of prevention. We work with low, moderate and high-risk populations within the community environment, bringing a variety of expertise and programs to support, produce, and maintain positive health outcomes. We do this through our programs and services:

Our delivery model is person centered, provider driven, medically integrated, and outcomes-based. Our team's experience, skills, and passion had grown as we worked with thousands of individuals with health risks, chronic disease and/or functional limitations. We create a unique delivery of health coaching and a model for transitioning patients from clinic to community. In addition, we developed Population Health Physical Therapy, a referral of individuals with chronic disease or health risk into evidence-based programs through an evaluation and exercise prescription by a physical therapist for safe and effective physical movement sustainable for a lifetime.
We believe strongly that to help individuals become more engaged and involved in their own health, we need to change the conversation with our patients, clients, providers, partners and stakeholders. CHP has made a commitment to provide evidence-based education and training on the process of behavior change and transforming the conversation through motivational interviewing techniques.
