The solution consists of a simple 3 step approach,
Step 1 - Early assessment - It takes less than 1 minutes to run an initial fall risk assessment which is used to find out if you have a low, moderate, or high risk of falling. Advanced assessments - 6MWT, 30SCS and TUG test can be performed and recorded within the solution.
Step 2 - Care Planning - If the assessment shows you are at an increased risk, the smart care planning is automatically populated with recommend interventions that can be implemented to prevent falls and reduce the chance of injury.
Step 3 - Seamless Collaboration - No more spending 30 to 60 minutes a day between shifts to update the status on different tasks or how the senior residents are doing. Everything is seamlessly handled right within the solution so that the information is always available.
