NHS Primary Care Trust responsible for ensuring residents of Hammersmith and Fulham have a full range of healthcare available to them. Established in April 2002. NHS Hammersmith and Fulham will improve the health of the local population. To support this vision we have four strategic goals. We use these to plan and prioritise work to deliver improvements in local people’s health and wellbeing. - Enable and support health, independence and well-being - Give people more control of their own health and healthcare - Offer timely and convenient access to quality, cost effective care - Proactively tackle health inequalities
