OnTrackNY is a network of Coordinated Specialty Care (CSC) teams across New York State that supports the well-being of teens and young adults impacted by early psychosis. OnTrackNY teams aim to empower participants to make meaning of their experiences and pursue their goals for school, work, and relationships.
OnTrackNY teams are guided by a centralized organization known as OnTrack Central. As a learning healthcare system, OnTrack Central is dedicated to ensuring that all OnTrackNY teams offer the best, most up-to-date care that science has to offer by maintaining a feedback loop between participants, families, team members, and administrators.
