Our Healthy Community is the connection between community-based organizations, community partners, and underserved community members. We were founded on the premise that equitable partnerships equal healthier communities. Through culturally specific engagement, data analytics, and strategic communication, we empower communities to mobilize and develop improvement plans to close the gaps that cause disparities.
Lack of timely access to both information and resources is a major social determinant of health. Faith-based and community organizations are uniquely positioned to disseminate information and resources that meet the needs of underserved community members. We empower community members to better advocate for themselves, their families, and their community.
