We develop and maintain relationships with community physicians and long-term care facilities so they can work together for the health and wellbeing of the patients We bring together physicians and long-term care facility that complement each other and in turn that will give the patients and families peace of mind knowing there is a cohesive relationship between the physician and the facility By getting to know the physicians and the needs of the facilities we are able to select a suitable physician/facility combination and that results in the most compassionate, respectful care of the patients
