In Project CHIP regio's werken zorgverleners ( huisarts, apotheker, medisch specialist, fysiotherapeut, POH, gemeente, thuiszrog etc.) transmuraal samen met 1 communicatietool naar patiënt (Inforium) en 1 gedeelde patiënt informatie database. (Project CHIP database).
Het project is een initiatief van de Stichting BetrouwbareBron en Mediwebs.
Project CHIP Almere is de voorbeeld regio. Gestart in mei 2016 bij het Flevoziekenhuis, De Kinderkliniek en de Zorggroep Almere en open voor andere organisaties en deelnemers. Met ondersteuning van VWS, Zorgverzekeraars Zilveren Kruis, CZ, VGZ. belangenorganisaties.
