Project CHIP ( Connected Health Information Platform) : De patiënt als portaal. In Project CHIP regio's werken zorgverleners ( huisarts, apotheker, medisch specialist, fysiotherapeut, POH, gemeente, thuiszrog etc.) transmuraal samen met 1 communicatietool naar patiënt (Inforium) en 1 gedeelde patiënt informatie database.
