The team of sixteen members provide assessment and intervention for any adult with a newly recognised or acutely exascerbated health or social issue, as part of the NHS 2-hour Urgent Response network. We can take referrals from GP, ambulance, voluntary sector, other NHS bodies, carers and family. We monitor new clients for around 3-4 days as longer term care is put into place for them. We comprise of nursing, physiotherapist, paramedic and social workers, and cover any client registered with a Telford & Wrekin (inc Newport) GP surgery for clinical interventions, and any who pays council tax to Telford & Wrekin Council for care interventions.
