Simplify

Our solution is Simplify, an AI and natural language processing-based tool, to be integrated into the EMR to improve communication and readability of patient-facing materials. Our goal is to reduce preventable hospital readmissions and empower patients to take control of their health outcomes as they navigate the health system- to better understand their medical conditions, medication dosing instructions, and the timeline for follow-up. Health Literacy disproportionately affects various groups, including 20% of adults who read at a 5th-grade reading level or below, Black, Hispanic, and Asian Americans, seniors and young adults, and Medicare and Medicaid population. Health literacy also affects our economy with 65% of preventable readmissions associated with poor health literacy. 50% of readmissions can be reduced with improvements to discharge instruction readability.
