Tech in Care is focused on delivering solutions for health and adult social care. Simply, Safely and Securely. Formed in March 2021 to drive forward the digital discharge to assess platform Hospital to Home (H2H) along with extensions into Intermediate Care and Interoperability – a Single Version of the Truth. Hospital to Home has since been rebranded to “Pathways to Care – Discharge to Assess”. The new name reflects our focus on technology-driven care solutions that enhance patient outcomes and improve healthcare delivery. Our purpose is to support Local Authorities, Integrated Care Boards and NHS Trusts' digitalisation strategies. Building tailored solutions to drive efficiences, whilst saving time, money and resource to effectively manage demand, improving the quality of life for people with health and care needs.
