The Rural Practice Network is an established Primary Care Network in South Somerset. We have five member practices - Millbrook Surgery, Queen Camel Medical Centre, Bruton Surgery, Wincanton Health Centre and Milborne Surgery. Together we manage the health of a registered population of around 37,000 people. We do this by improving healthcare inequalities, targeting gaps in service provision, encouraging people to pro-actively manage their own health and continuously improving services delivered by GP teams. We deliver many forward-thinking services in partnership, mostly recently running one of the most successful GP vaccination services in Somerset. We are fast-paced and are always ahead of the curve, brave enough to try new models of care. We are really proud of our MDT team - Pharmacists & Technicians, Paramedics, Health Coaches, Social Prescribing Link Workers, Care Coordinators and MSK Practitioners. The work they do is centered around the person, linking up healthcare and community provisions to get the best outcomes for people so that they stay well and achieve what matters to them.
