Transformative Consulting helps non-profit and community organizations to transform vision and ideas into meaningful planning and actions. We work with clients to leverage existing assets, capitalize on opportunities, and transform vision and ideas into purposeful growth and change. With experience in working with community health centers, public health agencies, neighborhood planning, social services organizations, and advocacy for transit, multi-modal active transportation, we work with entities to find the intersections of inter-linked issues; particularly those related to addressing the social determinants of health. We aim to connect the entities we work with to bring forth vision, generate ideas, connect concepts and issues, surface collaborative opportunities and identify and develop strategies to address potential barriers. Transformative Consulting offers numerous services to help organizations leverage their missions: * Lead strategic discussions and strategic planning * Use quantitative fact-finding and qualitative information gathering to conduct organizational performance analysis, community health needs assessments, and other approaches to inform business or service delivery planning and strategic planning. We also have extensive experience in convening and leading workshops and community forums, and developing successful grant proposals (with a specialty in federal proposal writing). Areas of concentration include working with Federally Qualified Health Centers (aka FQHCs or Community Health Centers), public health entities, state health associations, community planning and design agencies, and transit advocacy organizations. In term of proposal development and writing, Transformative Consulting has assisted community health centers to earn more than $80 million in federal grant funding to add new sites, grow services and programs, build and improve facilities and sustain access and services.
