Through our extensive dialogue with different stakeholders within hospital space, we've identified two pressing issues concerning hospital workflows a) Patient Transfer Management - PTM, and b) Chronic Patient Management - CPM.
PTM: Hospital network lacks an effective Patient Transfer Management to coordinate patient transfer amongst hospitals. In Germany, up to 700.000 patients are transferred between acute care hospitals per year. In the US, the transfer planning of a patient takes up to 18% of a doctor's working time.
CPM: The burden of chronic disease adversely affects the lives of tens of millions of patients and their families. Their lives are plagued by frequent exacerbations, often resulting in hospitalizations.
