SUMMARY / SCOPE:ASHAs or Accredited Social Health Activists are a community of health workers appointed by India's Ministry of Health and Family Welfare. These local women are placed in charge (one per village) to act as health educators and promoters in their communities. One of an ASHAs many responsibilities is to keep demographic records which can consist of population numbers and health record information. They are the key communication mechanism between rural populations and the healthcare system in India. The purpose of our app was to create an easy to use survey implementation tool that could be utilized by groups such as the ASHAs in better recording information rather than having to carry around stacks of paper from town to town.CONTRIBUTIONS:· Worked with a highly qualified team to come up with an efficient survey implementation to be used in third world countries to identify high risk patients.· Deployed a non-trivial medical questionnaire. Helped build the entire system from the ground up by simplifying the database schema. · Led quality assurance, testing / debugging, facilitating group meetings, and aiding inthe design of the user interface. Organized team presentation.· Project was picked up by Intermountain Healthcare.
