I am responsible for facilitating Individual Plan meetings and writing daily case notes for each person. During an Individual Plan meeting, I work with the treatment team and the individual to plan goals for the upcoming year. Also, I oversee how each provider will implement the Individual Plan by evaluating service delivery so as to ensure the best outcome for each individual. As well as communicating with the treatment team, I make sure that the proper paperwork is filed with the Developmental Disabilities Administration’s Regional Office regarding changes and continuations in services. As a graduate assistant I work closely with professors on a wide array of projects. I managed confidential student information and kept detailed student attendance records throughout the semester. Further, I have worked with professors on their research by independently accessing relevant literature and statistics. Making sure I remained organized in handling large amounts of literature and data was paramount and was something at which I excelled. Coding qualitative data as well as reading and understanding quantitative data were important functions of my position. I was responsible for assisting a 30 consumer caseload identify, and access, community resources during my time as a Blended Case Manger.
