Doctor Plus® è un percorso assistenziale in grado di adattarsi alle esigenze dei singoli clienti.La prima fase del servizio prevede il supporto al medico nella stratificazione dei pazienti più a rischio in base alla loro storia clinica.Successivamente il servizio aiuta i clinici nell’individuazione e nella formazione dei pazienti più idonei a cui offrire il servizio. A questi viene consegnato un kit di dispositivi medici certificati (classe IIa) per eseguire delle semplici misurazioni direttamente da casa. I valori dei pazienti vengono monitorati costantemente da una Centrale Operativa, composta da infermieri specializzati che, sulla base di un protocollo clinico condiviso, intervengono laddove si presentino degli alert clinici e svolgono una funzione di coaching e formazione a supporto dell’empowerment del paziente. Il medico viene coinvolto laddove il suo expertise è richiesto ma, soprattutto, ha la possibilità di vedere, direttamente dal suo gestionale i valori clinici dei propri pazienti giornalmente.I principali benefici sono:per il SSN un utilizzo più efficiente delle risorse sanitarie (minori accessi al pronto soccorso, diminuzione delle degenze in ospedale, utilizzo dei cosumabili etc.) ed un supporto concreto ad una gestione integrata tra Ospedale e Territorio;per il medico un innovativo supporto nella gestione quotidiana del proprio lavoro unito a migliori risultati clinici misurabili nel tempo;per il paziente un miglioramento nella gestione della patologia grazie ad una maggiore aderenza al proprio piano di cura e ad una migliore consapevolezza del proprio stato di salute.Tra le mie principali attivita':Customer Service Specialist, gestire un team di Infermiere delle chiamate inbound.Ticketing e CRM dedicato alle attivita' giornaliere.Gestione dei consumabili e del materiale informatico.
