As a Transitions of Care/Ambulatory Care Intern Pharmacist at UCHealth, I assist clinical pharmacy staff in providing efficient and optimal patient discharge, promoting positive health outcomes and reducing hospital readmissions. I collaborate with nursing staff and health teams to process patient medication orders, educate patients on their medications, and evaluate their medication adherence and compliance. I also act as an advocate for patient concerns and quality hospital experience, ensuring that their needs are met and addressed. I am a P3 Doctor of Pharmacy candidate at the University of Colorado Anschutz Medical Campus, where I am developing my knowledge and skills in pharmacy practice, pharmacology, therapeutics, and pharmacoeconomics. My goals are simple:- Simplify pharmacy for patients and maximize their reassurance, individual care, and well-being- Use my overgrowing passion for pharmacy to serve my community- Create positive patient outcomes through effective medication therapy management, patient counseling, and patient advocacy.
