Selected to be a member of the Strategic Group to determine future priorities with the staff team, the Quality Assurance Group which prioritises Enter & View visits to Health & Social care organisations and quality assures the reports produced by the E&V Teams, the Quality Account Group which influences local health care providers on the content of their Quality Accounts and monitors performance against their priorities.

North West

Acted as a part time Associate for Attain identifying commissioning support requirements from CCGs in the North West. Specialist areas-commissioning development, service redesign, quality improvement & assurance.

Chester

•Acted as a specialist resource on Quality Improvement and developing the Quality offering for the CSS.•Developed a Quality dashboard for the PCT Cluster to performance manage CCGs•Advised CCGs on how to improve outcomes in community services•Scoped 3rd sector organisations capability and interest in providing Commissioning support and exploring developing formal partnerships-Project managed CCG development of 111 Directories of Services on behalf of Cheshire, Warrington & Wirral Cluster

Salford

Role was to act as a Commissioning specialist resource to AQuA Executives and Programme leads. Liaising with CCG and CSU leaders across the North West to identify their commissioning development needs and recommending to the AQuA Executive Team and Board how best to tailor and market their products and programmes in a way that added value to the commissioning support services currently available. Role provided me with opportunity to work at a more strategic level and support Commissioning leaders across the North West during this time of unprecedented change.Achievements• Gathered intelligence from a range of CCG and CSU leaders in the NW to produce detailed recommendations for the AQuA Board on options for the membership offer for Commissioners for 2012/13• Developed a Commissioning for Outcomes programme for CCGs with the SHA Commissioning Development Team, incorporating Authorisation Dry Run development needs, which is being adopted by the DH for national roll-out• Developed a local CQUIN scheme and a QOF Quality Productivity (QP) template for Shared Decision Making which have been enthusiastically received by CCGs• Project managed the Observatory Team to develop Improving Outcomes Packs for COPD, Stroke and CKD and marketed them to Commissioners• Provided tailored Commissioning support to Commissioner members e.g. acted as ‘critical friend’ in development of QIPP Plans• Explained the AQuA products and programmes in a way that CCGs and CSU could understand the benefits and value of continued AQuA membership• Educated the wider AQuA Team on the Commissioning transition and the need to tailor products in a way that helps Commissioners achieve their key deliverables-authorisation and QIPP

Role required a collaborative, facilitative approach to achieve improvement through others, an ability to simplify complex guidance into workable local solutions and the use of patient stories to inspire and influence change.Achievements• Developed a high performing Quality Team that has successfully raised the profile of quality in the local health economy by identifying quality improvement priorities through the triangulation of data and ’soft’ intelligence. • Developed specialist knowledge of demonstrating and measuring quality improvement through contractual flexibilities - CQUIN and KPI’s.• Provided quality assurance of commissioned services for the PCT Board and GP Commissioning Consortia through the production of Quality dashboards for each main provider.• Developed specialist knowledge of how to demonstrate QIPP (Quality, Innovation, Productivity and Prevention) outcomes, LEAN methodology, transformational change and organisational development to support GP and PCT commissioners• Facilitated excellent collaborative working across Wirral Health and Social care organisations to improve quality and increase productivity through the development of system-wide quality outcome metrics for insertion in contracts to reduce harm and maintain patients in the community. Required creativity and OD skills in team building and patient and public involvement to create an innovative environment for service redesign to flourish and be clinically- led ensuring a strategic fit between PCT and PBC commissioning intentionsAchievements• Developed a highly commended (annual PCT award for Innovation) team of service redesign and Practice-based commissioning managers• Provided a strategic focus to service redesign so that practice-based commissioners commissioning plans reflected PCT and national priorities• Worked with Clinical leaders to deliver high profile redesigned system-wide models of care e.g. Integrated Care at Home for Long Term Conditions, integrated End of Life model• Ensured delivery of key care pathways supporting the shift of care from secondary to primary care and linked to Map of Medicine • Tested innovative ways of involving patients, carers and the public in redesigning services – patient videos, theatre groups• Led on the introduction of Practice-based Commissioning-developed practice incentive scheme, commissioning packs with locality profiles

Achievements• Won the Health Service Journal (HSJ) Secretary of State’s award for Innovation and the Long Term Conditions award in 2006 for designing and leading the implementation of the clinical champion model for three chronic diseases-CHD, diabetes and COPD to develop community based models of care. Evidence was provided to the judges of reduced hospital admissions and improved patient experience.• Led the development of a case management model in the community to prevent avoidable hospital admissions which was assessed by a US consultancy as the best in Cheshire & Merseyside• Led the redesign of whole system care pathways for long term conditions across the North Mersey footprint working collaboratively with primary and secondary care clinicians• Led the introduction of Practice-based commissioning consortia
