Current• Ability to effectively communicate with staff, supervisor, clients, mental health and social services agencies while utilizing the ACT model to support clients with their well-being and living with their mental illness.• Implement outreach to identify unserved, underserved, or inappropriately served adults and older adults with serious mental illness.• Maintaining strict confidentiality of client information and develop and implement accurate documentation of outreach outcomes.•Providing case management services, rehabilitation services, and teaching coping skills to support the clients with becoming more self-sufficient and independent despite their severe mental illness.•Linking clients to additional community, medical, and therapeutical resources to support their well-being and growth with living independently with their mental illness.
