Addictions Counselor
Current*Understands a variety of models and theories of addiction another problems related to substance abuse as reflected in treatment planning and clinical intervention.*Provides treatment services appropriate to personal cultural identity and language of the patient and family*Determines patient readiness for treatment and change as reflected in accurate assessment of the patient's stages of change and the use of interventions appropriate to each stage.*Establishes a helping relationship with patient displaying dignity and respect as reflected in professional language and appropriate boundaries.Identifies the psycho-social needs of the patient and family as reflected in the psycho-social assessment and the quarterly and annual evaluations.Development within 28 days of admission of a Master Treatment Plan which prioritizes the patient's needs and outlines measureable and attainable goals and action steps which are expressed in the language of the patient.Arranges referrals to other agencies, community programs and other resources to meet the needs of the patient and family as reflected in progress notes and case management notes.Documents assessment findings, treatment recommendations, patient interactions and progress as well complete all documentation within required time frame.Provides individual counseling Promotes knowledge of the disease concept of addiction and the process of treatment and recoveryFacilitates the development of basic life skills associated with the recovery process.Understands a variety of group methods/strategies appropriate to substance abuse treatment and can conduct group sessions which move toward identified goals.Prepares discharge planning for patients as reflected in complete and accurate Aftercare/Transition plans for patients.Conduct intake assessment interviews; ensuring all admission forms, releases, psycho-social interview, intake note and PCPC note are complete.
