As a Care Coordinator, I manage a case load of 400+ Medicare/Medicaid members with complex medical and behavioral health needs. As the main point of contact for all member issues, I complete Initial/Annual health assessments, follow up calls after hospital and emergency room visits, calls to ensure members are adherent to their medication regiment and respond to all incoming calls and messages. These calls ensure our members receive the most appropriate and highest level care available.

La Crosse, Wisconsin Area

As part of a semester long internship with the legal advocacy department of a domestic violence shelter, I assisted with follow up calls to the individuals that were listed as the victims on the city of La Crosse police reports that we received. I assisted with the filling out and filing of temporary restraining orders (TRO) and accompanied individuals, as a support system, to court hearings.

I assisted the home room teacher with classroom duties. I organized a stop smoking campaign that the students helped promote through out the school. We made T-shirts, handed out stickers and organized a "launch day" to celebrate the ban in indoor smoking on July 5th, 2010. At this launch the students handed out root beer floats, stickers, buttons and t-shirts to fellow students while talking to them about the dangers of smoking. I also organized a "girls group" where I headed up discussions of things that were stressors for the female students and helped them work through some of those issues.
