Oct 2016- March 2017 Developmental PositionDescription: Promote and provide elders and people with disabilities, services and programs which encourage independence and dignity offered in the least restrictive, cost effective, and appropriate environment. Assist medicaid long term service recipients in obtaining needed medical and related services through eligibility determination, program collaboration, and program referrals. Key responsibilities:Plan for and monitor long term care services.Develop comprehensive client directed, outcome-based care plans in conjunction with the client and/or their rep based upon the client’s need assessment. Arrange for the provision of care in a variety of care settings.Arrange for payment to care providers in a variety of care settings. Monitor the client’s needs risk status.Inspect the condition of residence to ensure safety standards are met. Collaborate with care providers and collateral contacts to assist with client needs and goals. Interview consumers, care providers, and collateral contacts to secure information necessary to determine program eligibility.Compile and evaluate facts to determine compliance with Medicaid, Medicare Savings Program, Title XIX, and SNAP program guidelines and regulations. Explain and interpret complex laws, rules, regulations, administrative rules, and policies to consumers and community partners. Conduct annual assessments (CAPS) to prepare and provide professional reports and recommendations to ensure consumer is receiving appropriate level of care and risks are mitigated. Send referrals to Adult Protective Services. Compile facts to support consumers in request for exceptions to program standards. Generate reports based upon inspection, observation, consumer request, and information obtain through interviews/investigations to assist consumer in requesting additional program supports.
