Mental Health Counselor
Current• Carry out immediate crisis intervention and counseling for patients and families• Complete emergent clinical evaluations of patient to aide in successful outcomes• Provide information, follow-up referrals and discharge planning to patients and families• Documents demographic and clinical information in the clinical record, complete all paperwork related to case management in accordance with quality assurance standards and written procedures• Provide prompt and thorough assessment, evaluation, and intervention via telephone or in person, and provide written and verbal communication where indicated• Work as part of a mental health emergency response team and avail themselves to work various shifts and covering call-offs when feasible The Crisis Staff will cooperate with all referral sources and will provide assistance, direction, and feedback as necessary and appropriate• Maintain current knowledge of community resources, how to access them, and the regulations for access to them
