    Aire Logic (formerly XML Solutions) were tasked with creating e-forms for use in Electronic Patient Record systems at Leeds Teaching Hospitals. To support this, I have developed a C# .NET based framework to integrate these forms into the Trust’s Electronic Patient Record system, PPM+. Approximately 6.4 million e-forms were submitted in PPM+ over the year to Sept 2019, providing improved searchability of patient records and eliminating the same number of paper forms with their associated environmental and administrative costs.I also worked on related projects including: • Forms supporting patient’s pathway through Yorkshire Lung Cancer Screening trial, including back-end calculations for cancer risk, and integration of cancer nodule predictions from CT scans via Veolity AI system.• Suite of ETL components to extract data from forms, for our Information teams.• Aggregated Summary View in PPM+, showing data trends across multiple documents.• Custom PDF design for Respect forms, for patients' resuscitation wishes.• e-Obs: Capturing and charting patient observations such as respiration, BP and heart rate.• Integration of Aire Logic’s new forms4health framework with improved form designer UI.
