I work in Barnsley Hospital in their Acute Medicine department, working across a 48 bed Acute Medical Unit, 27 bed Short Stay Unit and their Medical Same Day Emergency Care (SDEC) Unit. Alongside my daytime duties, I contribute to a full-time 1 in 11 Medical Registrar on-call rota over my 2 years of employment.I also frequently and independently perform practical procedures for patients in areas covered by the Acute Medicine team as above, namely lumbar puncture, ascitic tap and abdominal paracentesis.My job plan includes two sessions per week devoted to medical education, which I use both to actively tutor undergraduate medical students through bedside teaching, as well as contribute to facilitating on local simulation courses (for FY1 and FY2 doctors). I have also used this time to complete my Postgraduate Certificate in Medical Education.Out of hours, I work as a medical registrar (as the single registrar overnight and during weekday daytime from 2020-21, and with a second registrar or IMY3 from 2021-22), covering the Acute Medical Unit, inpatient medical wards, alongside referrals from Accident and Emergency or other inpatient specialties. I am also be a member of the Cardiac Arrest Team when on-call, and usually the designated Team Leader.

As the RCP Associate College Tutor, I had a role in education and leadership, namely aiding in reconfiguring the teaching programme to medicine junior doctors, and in discussions regarding the medical rota and training opportunities fo internal medicine trainees.This was particularly critical given the adverse effects on training prompted by the Covid-19 pandemic.
Core Medical Trainee (Ct2)
In my second year of Core Medical Training, I conducted four month rotations at Barnsley Hospital, Sheffield in Respiratory Medicine and a prolonged 8 month attachment in Cardiology (my Gastroenterology rotation was sadly cancelled due to the Covid-19 pandemic. I also was on the on-call SHO rota for medicine cover (between medical admissions, ambulatory care and general medicine ward cover). I worked during this time to consolidate my clinical experience, and start to make the step towards working as a medical registrar. I had a two month period during the pandemic to act up as a registrar, with adequate support.Extra Curricular Projects:- As the RCP Associate College Tutor, I had a role in education and leadership, namely aiding in reconfiguring the teaching programme to medicine junior doctors, and in discussions regarding the medical rota.- I rolled out the bedside teaching FOOT scheme to final year students in Barnsley and Doncaster, while continuing the project in Sheffield.
Core Medical Trainee (Ct1)
In my first year of Core Medical Training, I conducted four month rotations at Northern General Hospital, Sheffield in Geriatrics (with out of hours Acute Medicine cover), Renal Medicine (with out of hours Renal Medicine, Cardiology and Spinal Rehabilitation Cover) and Emergency Medicine. I used this time to consolidate my general medicine skills, develop experience in outpatient clinics and further complete examinations for my MRCP (UK) diploma.Extra Curricular Projects:- I rolled out the bedside teaching FOOT scheme to final year students in Sheffield Teaching Hospitals NHS Foundation Trust- I completed a Quality Improvement Project on IV fluid prescribing in Geriatric and Stroke Medicine.
Foundation Year 2 Doctor
During my second foundation year, I rotated through Clinical Oncology (with out of hours cover for Haematology and Palliative Medicine), Genitourinary Medicine (Community Sexual Health with out of hours cover for General Medicine and Acute Medicine) and Anaesthetics (providing cover for a Level 1 Surgical Step Down Unit). I was able to use these jobs to build my clinical decision and management skills, with a slant towards managing critically unwell patients, and patients approaching their end of life.Extra Curricular Projects:- I continued to co-ordinate the larger FOOT scheme run in Derby- I completed an audit on Fast Track discharge letters, and worked with a team to suggest interventions to provide improvements in teaching staff on writing these.
Foundation Year 1 Doctor
I rotated through Gastroenterology, Colorectal Surgery and General Psychiatry during my time of provisional GMC registration. I used this clinical experience to develop myself into a safe and competent clinical practitioner. Extra-Curricular Activities:- I co-ordinated a teaching scheme locally ("FOOT"), allowing for weekly bedside teaching delivered by FY1 doctors for final year medical students.- I participated in data collection and analysis for two audits during my time in Chesterfield; a regional audit during my placement in Gastroenterology related to the management of upper gastrointestinal haemorrhages, the second by collecting follow-up data on post-operative acute kidney injury for a national audit.
