Current Provide services and education to dialysis patients and caregivers to support and maximize patient nutritional well-being. Develop individualized Medical Nutrition Therapy for each patient. Develop education materials and provide nutrition education and counseling based on individualized patient needs. Monitor nutritional status, laboratory values, adherence to diet and response to nutrition related medications to determine appropriate interventions and recommendations for all dialysis modalities (Incenter, Peritoneal, Home Hemodialysis, and Nocturnal). Evaluate outcomes and identify factors that may contribute to undesirable results and document nutrition progress monthly. Conduct facility in-services on anemia management and mineral and bone disorder. Preceptor for new hire dietitians to DaVita and intern coordinator.
