• Provide ongoing case management, community support services and outreach services to service recipients • Maintain appropriate contact with other providers and referral sources including providing• information on consumer’s progress, behavior, and needs• Provides facility-based rehabilitative skill-building services for adults 18 years of age and older with serious mental illness or co-occurring psychiatric disabilities in an individual and/or group setting.• Perform and initiate planning for service delivery to each assigned client by coordinating and assisting the client with receiving necessary services, advocating for and supporting the client in obtaining resources•

• Document all care and treatment the child receives• Provide reassurance and direct observation of assigned child• Promptly and effectively communicates with patients, family, visitors, nurses and physicians• Report the child’s behaviors on a timely basis• Assures a safe environment and notifies hospital staff of concerns

• Operate crisis hotline calls by presenting information and resources to assist victims• Provide effective and timely assistance to victims including crisis intervention, referrals for long-term counseling issues and other basic needs, coordinate provision of services with case managers in accordance to defined advocacy model of service• Co-facilitate support groups for adult and youth survivors that are victims of, or have been exposed to, domestic violence. Manage accurate and up-to-date documentation of phone calls, contacts, referrals, court information, etc.• Participate in all appropriate center meetings and training sessions• Maintain current professional knowledge of domestic violence and crisis theory, including assessment of danger and resources of direct solution

• Accurately maintain 500 client case files and statistical data within the Medicaid and Medicare population• Manage a team of 20 case managers to provide crisis intervention, intensive long-term case management, advocacy and aftercare services • Prioritize visits of clients by therapist and case managers• Collaborate with health care providers for the development of care with the client that is based on client goals, needs and issues (e.g. substance abuse, mental illness, medical problems, etc.)• Assess and refer clients to appropriate social services agencies • Organize and attend internal/ insurance providers staffing meetings providing statistical data and updating clients’ case file• Develop effective, trusting and professional relationships with health plans and payers• Acquire authorizations for services from health care providers
