- Provided MTM services in the HealthPartners Riverway Anoka Clinic utilizing multiple CPAs including hypertension, diabetes, lipids, asthma, copd, smoking cessation, and thyroid- Provided clinical services and MTM in Goodrich Pharmacies through HealthPartners Slice of PIE program, MedWise, Outcomes, and Docstation-Created a Metformin IR to XR switch protocol and statin therapeutic interchange protocol approved by local providers for Goodrich pharmacy- Implemented a new program for tobacco cessation at Goodrich Pharmacy by starting the nicotine replacement therapy protocol through MN BOP-Acted as a clinical preceptor to 10 APPE students during their ambulatory care rotations: organized journal clubs, topic discussion, taught students how to lead clinical MTM visits and write SOAP notes- Consulted for long term care facilities-Organized and ran adult and pediatric Covid and influenza clinics, providing over 1,500 Covid vaccines and 650 influenza vaccines over the 2021-2022 year- Staffed Goodrich pharmacies once weekly and every third weekend, while answering clinical questions daily during staffing from providers in the HealthPartners clinics
