QIDP (Qualified Intellectual Disabilities Professional)- Responsible for providing supervision, assisting with program development, and implementation of JDT services through Desert Regional Center (DRC).- Perform case management duties for individuals with intellectual and/or physical disabilities.- Meet with individuals periodically and ensure their health and safety needs are being met.- Develop, implement, and train staff on plans specific to the individual being served.- Track and monitor progress, create quarterly and annual reports, and participate in annual PCP (Person Centered Plan) meetings.- Create and submit incident reports following proper protocols.- Communicate with DRC Service Coordinators, family members, caretakers, and state employees where appropriate.- Train new and existing staff on DRC policies and procedures, Nonviolent Crisis Intervention, and other essential trainings.Intake Coordinator- Process all inquires for enrollment of a loved one for Adult Day Care services and schedule tours.- Review and determine eligibly for enrollment.- Responsible for processing necessary paperwork for all potential admissions, including, but not limited to Inquiry Forms, Physical Evaluation Forms, Transportation Forms, and Admission Packets.- Submit monthly reporting and ad-hoc reports as requested.- Determine payment and insurance eligibility using various systems and working with outside organizations.- Provide information and resources to family and other caretakers.
