Managed all training & development needs for 90+ employees. Worked closely with Human Resources Department. Developed and implemented annual training plan.

Supervised 3 employees in grant funded program providing medical/behavioral health care management to high need consumers in 8 counties. Created monthly reports for funders. Facilitated and co-facilitated bi-weekly staffing meetings with contracted psychiatrist and behavioral health certified pharmacist. Hired and trained new staff. Liaison for hospital and mental health agencies. Supervisor for 5 care managers working with uninsured individuals in 8 counties.

PrimeCare Care Manager - worked on a 2 year study of the affects of care management on patients with depression. Medical Care Manager - Worked with individuals signed up for Medicaid with chronic medical conditions to assure linkage to medical home, understanding medical instructions including medications, and supporting the appropriate use of emergency services.Behavioral Health Liaison - Managed a grant funded program working with medical practices in implementing time based protocol for follow up with patients with new diagnosis of depression. Contracted with consulting psychiatrists & practices for grant. Maintain spreadsheet and created quarterly reporting. ICARE Coordinator/Care Manager - Provided direct care management services to patients with severe and persistent mental illness and chronic medical conditions. Facilitated and co-facilitated monthly meetings. Created monthly and quarterly reports. Supervised 1 employee.
