Assisting resident relations and social services managers in independent living, assisted living, and memory care. Collaborating with transitions team, conducting wellness check ins, and providing support for residents during the time of COVID-19.

• Provides 1:1 case coordination and case management to students at-risk for truancy • Conducts Global Appraisal of Individual Needs (GAIN) screenings, risk assessments, and Biopsychosocial assessments with youth in truancy program • Assesses suicide risk and follows agency and school district protocol • Collaborates with multidisciplinary team to support students and families and refer to other services • Counsels students and provides interventions to work towards goals• Assists with teaching classes to youth and families including Cost of Crime, Family Empowerment, and youth attorney education • Facilitates strength-based atmosphere, positive social skills, and trauma-informed practice with students and families• Utilizes Subjective, objective, assessment, & plan (SOAP) method of documentation and tracks student attendance and grade data for state grant • Supports peer court program in conducting interviews and intakes with youth and families, assisting with recruitment and training of youth attorneys and jurors, and by providing support to youth volunteers and participants during peer court proceeding

• Provided in-home, community, and school-based skills training, counseling, and case management to diverse caseload of youth• Collaborated with multidisciplinary team to support youth in treatment and for case consultation • Recognized need for social skill and coping skill groups within the agency, proposed and developed curriculum for groups, and co-led small groups for youth in treatment• Oversaw Children’s Case Manager duties as needed including child mental health hospitalizations, referrals for sub-acute and residential care, and discharge planning• Assisted in dispatching crisis response, assessed client safety, and communicated assessment to licensed mental health professional for follow up • Documented clinical notation in electronic health record systems Credible and Profiler
