Director, Adult Family Care
Current-Manages and coordinates the hiring, training, supervision, evaluation, and development AFC case managers and registered nurses. -Oversees personnel management and resolution of employee relations issues, in conjunction with the Human Resources department, including (but not limited to) creation and execution of corrective action and performance improvement plans, as appropriate. -Monitors and creates systems to drive census growth and ensure the financial viability of the AFC program, including (but not limited to): • Collaboration with internal leadership and development staff to improve marketing efforts. • Development, maintenance, and expansion of a network of referral sources. -Ensures compliance with regulatory requirements -Oversees employee caseloads to ensure efficiency, effective service delivery, and alignment with industry best practices. -Actively participates in the AFC Council and keep abreast of industry standards and best practices. -Oversees intake process and ensure the appropriateness and smooth execution of all AFC placements, including in-person coverage of intake-related visits, as needed. -Manages reduced caseload as needed to ensure proper onboarding, case assignment and caseload management.
