I am the person responsible for following up within 48 hours with each client upon their discharge to check in and ensure they are aware of follow up appointment information. I log and document these calls in an electronic health record. I also link clients to internal and external services and resources. In addition to these duties, I am the case manager for crisis unit, so I take on the high acuity clients and work with them following their discharge to ensure a smooth transition and to help them get on their feet. I facilitate psycho-educational groups. I also help clients get into programs such as substance use programs.

I helped clients get connected to community resources and internal and external resources. I assisted clients in making and keeping up with appointments. I facilitated skill building groups such as anger management and Women's Workbook for Addiction. I helped reunite women with their children after a DFCS/CPS case. I documented everything in an electronic health record.

This was a community based, first episode psychosis treatment team position. That means that we went out into the community to meet with clients, often at their homes. I conducted individual sessions with clients to help them learn and practice new skills. I facilitated peer support groups.
