• Support individuals living with disabilities to ensure health, safety and continuation of their goals to live independently• Assist with housing procurement, as well as the acquisition, retention and improvement of skills including social and adaptive skills and personal finances

• Complete psychosocial assessments and treatment plans • Facilitate group therapy and 1:1 patient meetings• Engage in treatment and discharge planning• Provide therapeutic and educational interventions• Document daily progress and clinical notes• Connect with family and social support for collateral details• Collaborate with multidisciplinary treatment teams including MDs and RNs• Assist in connecting individuals to resources and outpatient services for continuing care

• Care for adult patients with psychiatric disorders, mental health issues, and substance abuse problems• Facilitate group therapy, psychological education groups• Populations included active duty military, dual diagnosis, geriatric, and crisis stabilization

• Acquisition and transfer of patient records to appropriate parties, including agencies and medical facilities under HIPPA compliance

• Facilitate group discussions and help residents with daily needs including attainment of resources
