I've have been an LPN for the last 20 years. My first 10 years were in long-term care and the past 10 years have been with Ambulatory Care/Family Medicine with Parkview Health. I have had multiple roles in my years with Parkview ranging from floor nurse, Site Supervisor, Clinical Supervisor and phone triage and Transitional Care Management. I have been a part of transitioning our 3 family medicine clinics to receive Rural Health accreditation. I have also helped mainstream our Transitional Care Management (TCM) process to help improve the overall health and follow up for our patients while also being able to see an increase in revenue with the TCM process.
