As a Personal Health Coordinator with Humana at Home I assist members in meeting their health and wellness goals, including physical, mental, financial, and social needs. Bringing Science Home is a chronic disease research project funded by The Patterson Foundation at USF Health that aims to help people with chronic disease live optimistic and empowered lives. The project focus begins with diabetes, but seeks lessons that can be applied to other chronic conditions as well.
