As social services director at one of the top rated nursing homes in Northern California, I was a medium between patient care and patient wants/needs. Everyday involved meetings to coordinate care and ensure that their medical needs, physical therapy needs, billing needs, social needs and discharge realities were all in line. Thorough medical documentation was filed on a daily basis regarding the patients. I also ensured any complaints from residents or staff were addressed, documented, and resolved. I supported nursing and physical therapy staff to better understand and therefore care for their patients. In addition to being a constant presence around the facility, I also transported patients to appointments.Before this I managed staffing of this 80+ bed skilled nursing facility during extremely challenging COVID, influenza and C. diff outbreaks.
