Direct, supervise, and train a team of up to 5support staff.Conduct weekly supervision meetings with direct support staff to ensure adherence to the expected job responsibilities, provide feedback, and lendsupport; Provide coverage support for case managers within the program, as needed. Facilitate trainings for case support and case manager staff related to case management services for client's educational, therapeutic, medical, and legal needs. Maintain and update any documents or forms, and Refugee Foster Care Manual, as necessary to ensure continuouscompliance; Create and implement new staff trainings for case support and case management staff, as needed.

Complete intake and assessments of potential participants, maintain case records on each participant, includes assessments, a social history, action plan, progress notes, and other supporting documentation. Track outcome measures in a timely manner, provide monthly statistical information to the Director of Social Services, Corps Officers, DHQ, and THQ leadership as necessary. Address the homelessness, income barriers and employment needs of the participant. Responsible for ensuring that program participants have access to mainstream health and social service programs such as TANF, Medicaid, State CHIP, SSI, Food Stamps, WorkForce Investment Act, and Veterans Healthcare programs. Assist participants to enhance personal support systems. Conduct weekly groups to address and equip the homelessness population. Assist in identifying and teaching skills in parenting, budgeting, conflict management, Domestic Violence, substance abuse awareness, educational advancement and job readiness.

Managed a caseload of people who have servere mental illness. Assisted consumers with gaining access to financial, housing, medical, social, vocational, educational, leisure and mental health services. Support consumers to their Psych and medical appointments. Act as an advocate for consumers at the Public Assistance Office and SS offices as well as provide translation as needed. Document interactions, medication and possible risk. Conduct intake and possible referrals. Provide quarterly reports, keep a record of time sheets and record mileage weekly. File paperwork in charts. Performed data entry medications, address change etc. Fax, mail, and scan paperwork to clinics, hospitals, lawyers, Housing representatives and Assisted Living facilities.
