I managed logistical aspects like setting up accounts, coordinating meetings, and ensuring new hires were integrated into our communication platforms, such as Slack and Team Channels, for daily interactions.Additionally, I collaborated across departments, working with product management to improve our Care Team Application and supporting enrollment teams with training on new releases. I provided ongoing support and mentorship, conducted quality reviews, and identified trends in questions, incorporating these insights into our training materials. My focus was on optimizing training processes, enhancing team performance, and ensuring high standards of care and productivity within the care team.

In my role as a Patient Care Navigator, I managed a substantial patient case load of 280-300 individuals monthly, focusing on aiding patients with their chronic conditions. I facilitated the creation of personalized care plans and health-based goals by collaborating closely with each patient, aiming to optimize their individual well-being. Moreover, I played a crucial role in ensuring effective communication between patients and clinicians during critical situations, ensuring timely and appropriate healthcare responses.Beyond patient care, I navigated multiple Electronic Medical Records (EMRs) to facilitate seamless care coordination across various teams and clients. I enrolled patients in chronic care management programs and provided insightful explanations about Medicare cost-sharing options, assisting patients in understanding and navigating healthcare expenses.
