I have recently relocated to my home state of Western Australia and am seeking a role in which I am able to use skills from my most recent employment position of Home Support Assessor with Regional Assessment Service (RAS). The role required me to identify services required by elderly clients to remain in their own home by completing the National Screening and Assessment Form, creating a support plan and communicate identified needs and linking clients to community organisations. Prior to the RAS position, I completed a Certificate IV in Individual Support whilst working in a community support role.
